After we select a user (i chose admin in my case) there will be a confirmation shown: [the password that we set during installation] Default login for omv 4, omv 5 web gui. I forgot that i got the default password changed in omv, so it seems i locked myself out using default password. I can enter web insterface by admin account. 1 login to console or via ssh. No idea about the raid setup though. Sometimes it just starts working again with no input.

Connect to your linux server via ssh or local access. Select the reset failed login attempt counter menu item: I'm currently having the same issue with an omv5 that was good yesterday but can't log into the gui today and an omv4 machine that i thought i had done something to that i can't log into the gui since this morning. This meant i had to reset the web admin password. Omv's ip will be there as 192.168.2.xx. You have to set up form fields. No setting required, works ootb if you set up the name of the entry correctly. I have 2 mates ive put onto omv originally, and both have suffered similar problems. Unfortunately these command does not fix it permanently & need to rerun it after each reboot.
